To be considered for this scholarship, you must be admitted as a matriculated (degree-seeking) student. An Application for Undergraduate. Admission is included in the University’s Application Materials packet, available from the University of Utah, Admissions Office, 201 S. 1460 E., Rm 250S, Salt Lake City, UT 84112-9057, (801) 581-7281. December 15 is the preferred filing date for admission to the University of Utah.
Eligibility
This scholarship is Available to Utah residents who are: continuing students, freshman and transfer students, who have declared a major in Anthropology OR Health, Society & Policy with an overall U of U GPA of 3.5, transfer students without U of U credit, or High School if incoming Freshmen.
Application Procedure

- Submit an official academic transcript from all colleges attended if you are a college student (waived if less than 15 transfer hours), or from your high school if you are an entering freshmen.
- Submit a 500 word personal statement concerning your educational and career plans (your exciting story of where you’ve been, where you want to go and why) and how receiving this scholarship would help you.
- Submit 2 letters of recommendation on LETTERHEAD, SIGNED and DELIVERED in a SEALED ENVELOPE with the AUTHOR’S SIGNATURE OVER the SEAL. (one must be academic) Do not use forms.
